Registered Nurse - Care Management - Community Healthcare
<p><strong>SUMMARY</strong></p><p>The role of RN Care Management is to work collaboratively with providers and the care team to provide safe, efficient and effective care to empaneled patients. Under the clinical direction of a departmental provider, the RN (1) facilitates health and disease management, (2) uses evidence-based care to improve quality and patient outcomes, and (3) assists in transitions of care. </p><p><strong>Duties and responsibilities may include but not be limited to:</strong></p><p>Facilitate timely patient flow to ensure provider interruption during clinical hours is kept to a minimum.</p><p>Demonstrate appropriate judgment skills to make independent clinical decisions using Clinical Practice Guidelines to respond to electronic triage messages and promote same day access to care for established patients.</p><p>Adhere to the scope of practice for a Registered Nurse according to A.R.S. §32-1606 and §R4-19-301 of the Arizona State Board of Nursing and promote the level of clinical expertise required to provide safe, high quality nursing care.</p><p>Monitor electronic communications assigned to providers and assist in timely responses and actions</p><p>As directed by the departmental provider, contact patients with abnormal lab/diagnostic results and facilitate required actions.</p><p>Attend all codes, provide appropriate care and transition of care.</p><p>Facilitate transitions of care, perform follow-up calls for patients who are transitioned to the emergency department, and ensure appropriate provider follow-up/documentation is established.</p><p>Perform appropriate clinical documentation to maintain the standards set.</p><p>Remain flexible and responsive when changes occur in workload.</p><p>When needed, assist with preventive care, health education, medication reconciliation, self-management goals, and quality improvement initiatives.</p><p>Provide timely productivity/utilization documentation to the Operations Director, as requested.</p><p>Performs other duties as assigned.</p><p><strong>ADDITIONAL RESPONSIBILITIES</strong></p><ol><li>Promotes positive patient/guest relation in accordance with the Health Centers policies, providing a high level of quality in personal attention and service to patients and visitors.</li><li>Observes organizational policies concerning unscheduled absences and reports unscheduled absences in accordance with departmental procedures. Have 3 or fewer occurrences of unscheduled absences in a twelve month period.</li><li>If in a nonexempt position, does not incur excessive overtime. Does not work overtime without prior written approval of his/her supervisor. If in a supervisory position, controls the overtime expended by his/her subordinates within the policies and practices of the facility.</li><li>Complies with the organization Safety Policies and Procedures. Ensures compliance with governmental licensing and regulatory requirements where applicable. Conducts annual review of and complete/suggest on-going revisions of the Policies and Procedures specific to his/her department. Maintains familiarity with and abide by all established facility and departmental policies and procedures, noting changes when they are published/posted. Conducts himself/herself in a manner consistent with the Mission Statement and Values.</li><li>Reports to work at the start of the shift 98% of the days scheduled. Follows the work rotation schedule. Explains and gives proper notification of absenteeism, as outlined in the facility Policies and Procedures. Attends a minimum of 98% of the staff meetings. Reads and signs off on all minutes of the meetings not attended. Participates in committees as assigned. Maintains 100% attendance at mandatory skill/departmental updates. Seeks out facility or external education/training that would further develop individual skills, experience or knowledge necessary for a high standard of functioning in his/her job.</li><li>Observes and respects the confidentiality of information in regard to patients, visitors and co-workers employees including salary information, if exposed to any of the above referenced information in the course of his/her job function.</li><li>Dresses according to the organization’s dress code policy and procedure, as well as, complies with specific departmental regulations pertaining to employee appearance.</li><li>Demonstrates an understanding of facility organizational structure by utilization of appropriate channels of communication. Pediatrician Opening - Central Phoenix
<p>Pediatric/Family Medicine group in Central Phoenix is seeking a Pediatrician. Newly expanded practice is seeking an additional provider due to patient demand. Current provider is seeking on average 30 patients per day. New provider will be required to see 15 patients per day, though grow over time. Occasional Saturday coverage is required, though will new provider can take a weekday off to offset the additional hours. Outpatient only with no inpatient rounding. Pay is negotiable with great incentives and benefits. Bi-lingual (Spanish preferred) provider preferred, though not mandatory. Great opportunity in a great location.</p>
